REGISTRATION IS LIVE! Sign up for Five County 4-H Summer Camp at Camp Lakotah in Wautoma for a dazzling camp experience on June 7-10th! This is a great chance to challenge yourself to new and exciting activities like canoeing, service learning, crafts, and more all while making friends across five counties!

Registration Information:

Online Registration Form

The online registration process will be completed using a Qualtrics Form (Similar to Google Forms) this year. Please submit a new form for every camper that plans to attend Five County 4-H Summer Camp. You should receive an email once your registration is received. Registration is NOT COMPLETE until payment is received. Please reach out to the Extension Office if you are having issues with completing the registration.

Paper Registration Form

The paper registration form is available as a view-only Google Doc. You must print the form to fill it out and turn it in to the Green Lake County Extension Office. If you need to add additional campers, please use the back of the last sheet. You may email scanned or photos of the registration forms in – please check the quality to make sure we can read the forms.

Payment

There are three (3) ways you can complete payments for Five County 4-H Summer Camp. Make checks payable to “Green Lake County 4-H Leaders’ Association.”

  • Venmo
    • Send payment to @GreenLakeCountyLeaders’-Associa. Put “Last name – 4-H Camp” in the “What’s this for?” section.
  • In-person at the Extension Office
    • Bring cash or a check to the Extension Office.
  • Mail
    • Checks may be mailed into the Extension Office!
