Camp Information

Five County 4-H Camp will be returning to Camp Lakotah in Wautoma for a groovy time on June 8-11th! This is a great chance to challenge yourself to new and exciting activities like canoeing, service learning, crafts, and more!
This year’s theme is “Never Underestimate the Power of 4-H’ers!”

Registration
Registration for Five County 4-H Camp is LIVE! Registration and payment must be received by the Extension Office by April 15th to be considered registered.
Registration Forms: There is both an online registration form and as well as a paper version. The forms are VERY similar, however one has form fields to type in your information and the other has underlined spaces for writing.
Payment: All payments must be turned in by mail or in-person at the Extension Office. Please make checks payable to the Green Lake County 4-H Leaders’ Association.

Online Registration Form
This registration form is designed with form fields to completed easily online. All you need to do is fill out your campers information on the form. Once you are finished, just save the form and email the completed copy to brandon.springer@wisc.edu.
Paper Registration Form
This registration form is designed to be printed and completed by hand. This is a great option if you plan to mail your registration form in or have issues completing the online registration form. If you are unable to print your own version, please contact the Extension Office for assistance.
